    Supporting children and young people to assume responsibility from their parents for the self-management of their long-term condition: An integrative review

    © 2019 John Wiley & Sons Ltd Background: Children and young people with long-term conditions (LTCs) are usually dependent on, or share management with, their families and are expected to develop self-management skills as they mature. However, during adolescence, young people can find it challenging to follow prescribed treatment regimens resulting in poor clinical outcomes. Though reviews have looked at children's and parents' experiences of self-management, none have explicitly examined the parent-to-child transfer of self-management responsibility. Methods: An integrative review was conducted with the aim of exploring the parent-to-child transfer of LTC self-management responsibility, through addressing two questions: (a) How do children assume responsibility from their parents for self-management of their LTC? (b) What influences the parent-to-child transfer of this responsibility? Eight databases were searched for papers published from 1995 to 2017. Methodological quality was assessed; included papers were synthesized to identify themes. Results: Twenty-nine papers were identified. Most papers used qualitative designs and focused on children with diabetes. Participants were predominantly children and/or parents; only two studies included health professionals. Assuming self-management responsibility was viewed as part of normal development but was rarely explored within the context of the child gaining independence in other areas of their life. Children and parents adopted strategies to help the transfer, but there was limited evidence around health professionals' roles and ambivalence around what was helpful. There was a lack of clarity over whether children and parents were aiming for shared management, or self-management, and whether this was a realistic or desired goal for families. Multiple factors such as the child, family, social networks, health professional, and LTC influenced how a child assumed responsibility. Conclusions: Evidence suggests that the parent-to-child transfer of self-management responsibility is a complex, individualized process. Further research across childhood LTCs is needed to explore children's, parents', and professionals' views on this process and what support families require as responsibilities change

    Comparative grazing behaviour of lactating suckler cows of contrasting genetic merit and genotype

    peer-reviewedThe objective of this study was to determine if differences in grazing behaviour exist between lactating suckler cows diverse in genetic merit for the national Irish Replacement index and of two contrasting genotypes. Data from 103 cows: 41 high and 62 low genetic merit, 43 beef and 60 beef x dairy (BDX) cows were available over a single grazing season in 2015. Milk yield, grass dry matter intake (GDMI), cow live weight (BW) and body condition score (BCS) were recorded during the experimental period, with subsequent measures of production efficiency extrapolated. Grazing behaviour data were recorded twice in conjunction with aforementioned measures, using Institute of Grassland and Environmental Research headset behaviour recorders. The effect of genotype and cow genetic merit during mid- and late-lactation on grazing behaviour phenotypes, milk yield, BW, BCS and GDMI were estimated using linear mixed models. Genetic merit had no significant effect on any production parameters investigated, with the exception that low genetic merit had a greater BCS than high genetic merit cows. Beef cows were heavier, had a greater BCS but produced less milk per day than BDX. The BDX cows produced more milk per 100 kg BW and per unit intake and had greater GDMI, intake per bite and rate of GDMI per 100 kg BW than beef cows. High genetic merit cows spent longer grazing and took more bites per day but had a lower rate of GDMI than low genetic merit cows, with the same trend found when expressed per unit of BW. High genetic merit cows spent longer grazing than low genetic merit cows when expressed on a per unit intake basis. Absolute rumination measures were similar across cow genotype and genetic merit. When expressed per unit BW, BDX cows spent longer ruminating per day compared to beef. However, on a per unit intake basis, beef cows ruminated longer and had more mastications than BDX. Intake per bite and rate of intake was positively correlated with GDMI per 100 kg BW. The current study implies that despite large differences in grazing behaviour between cows diverse in genetic merit, few differences were apparent in terms of production efficiency variables extrapolated. Conversely, differences in absolute grazing and ruminating behaviour measurements did not exist between beef cows of contrasting genotype. However, efficiency parameters investigated illustrate that BDX will subsequently convert herbage intake more efficiently to milk production

    Bringing self assessment home: repository profiling and key lines of enquiry within DRAMBORA

    Digital repositories are a manifestation of complex organizational, financial, legal, technological, procedural, and political interrelationships. Accompanying each of these are innate uncertainties, exacerbated by the relative immaturity of understanding prevalent within the digital preservation domain. Recent efforts have sought to identify core characteristics that must be demonstrable by successful digital repositories, expressed in the form of check-list documents, intended to support the processes of repository accreditation and certification. In isolation though, the available guidelines lack practical applicability; confusion over evidential requirements and difficulties associated with the diversity that exists among repositories (in terms of mandate, available resources, supported content and legal context) are particularly problematic. A gap exists between the available criteria and the ways and extent to which conformity can be demonstrated. The Digital Repository Audit Method Based on Risk Assessment (DRAMBORA) is a methodology for undertaking repository self assessment, developed jointly by the Digital Curation Centre (DCC) and DigitalPreservationEurope (DPE). DRAMBORA requires repositories to expose their organization, policies and infrastructures to rigorous scrutiny through a series of highly structured exercises, enabling them to build a comprehensive registry of their most pertinent risks, arranged into a structure that facilitates effective management. It draws on experiences accumulated throughout 18 evaluative pilot assessments undertaken in an internationally diverse selection of repositories, digital libraries and data centres (including institutions and services such as the UK National Digital Archive of Datasets, the National Archives of Scotland, Gallica at the National Library of France and the CERN Document Server). Other organizations, such as the British Library, have been using sections of DRAMBORA within their own risk assessment procedures. Despite the attractive benefits of a bottom up approach, there are implicit challenges posed by neglecting a more objective perspective. Following a sustained period of pilot audits undertaken by DPE, DCC and the DELOS Digital Preservation Cluster aimed at evaluating DRAMBORA, it was stated that had respective project members not been present to facilitate each assessment, and contribute their objective, external perspectives, the results may have been less useful. Consequently, DRAMBORA has developed in a number of ways, to enable knowledge transfer from the responses of comparable repositories, and incorporate more opportunities for structured question sets, or key lines of enquiry, that provoke more comprehensive awareness of the applicability of particular threats and opportunities

    Support for the Right Hemisphere Hypothesis of language processing: An investigation of ambiguous word resolution in puns

    Right frontal hemispheric stroke causes cognitive difficulties that include loss of appreciation of verbal humour (Shammi & Stuss, 1999). Although nonverbal creativity and working memory have been linked to this impairment, a deficit in the coordination and comprehension of ambiguous verbal material is likely to playa significant role. In this way, the Right Hemisphere Hypothesis of language processing (Coltheart, 1987) might contribute a plausible explanation for deficits in humour appreciation post-stroke, which would inform models of normal language processing. Through a series of four experiments, the current study contributes knowledge regarding the hemispheric specialization of processing puns. Puns were chosen for their propensity to force dual ambiguity resolution in a humourous context. Results from a single-word lexical decision task demonstrated priming for dominant associates of ambiguous targets. A centralized lexical decision task with pun primes and dominant, subordinate, and unrelated targets showed strongest priming for dominant relatives. A divided visual field study revealed that at 500 ms ISI, both hemispheres activated, but the left activated in such a way as to suggest that its pattern was driving the results for the centralized study. In contrast to the lexical decision data that favoured the dominant targets, data from a forced-choice relatedness task showed an advantage for the subordinate associates. Results from this series of experiments provide a working model of how puns are processed in neurologically intact individuals and contribute to the body of literature supporting the Right Hemisphere Hypothesis of language processing

    Large-area sheet task advanced dendritic web growth development

    The thermal models used for analyzing dendritic web growth and calculating the thermal stress were reexamined to establish the validity limits imposed by the assumptions of the models. Also, the effects of thermal conduction through the gas phase were evaluated and found to be small. New growth designs, both static and dynamic, were generated using the modeling results. Residual stress effects in dendritic web were examined. In the laboratory, new techniques for the control of temperature distributions in three dimensions were developed. A new maximum undeformed web width of 5.8 cm was achieved. A 58% increase in growth velocity of 150 micrometers thickness was achieved with dynamic hardware. The area throughput goals for transient growth of 30 and 35 sq cm/min were exceeded

    The non-linear zero-differential resistance state (ZDRS) that occurs for highly mobile two-dimensional electron systems in response to a dc bias in the presence of a strong magnetic field applied perpendicular to the electron plane is suppressed and disappears gradually as the magnetic field is tilted away from the perpendicular at fixed filling factor ν\nu. Good agreement is found with a model that considers the effect of the Zeeman splitting of Landau levels enhanced by the in-plane component of the magnetic field.Comment: 4 pages, 4 figure

    Computational mechanical analysis of composite bondlines under environmental moisture loads

    AbstractA combined moisture diffusion and mechanical computational analysis methodology was developed as part of an independent research program at Lockheed to enhance our understanding of nonlinear and coupled physical processes in materials. The physical behavior of interest is the mechanical response of composite material systems to environmental conditions which can change the materials' moisture content. In particular, it is desirable to know what set of environmental conditions will affect the integrity of a partial adhesive bondline when a bondline gap (no adhesive) is present. The Lockheed proprietary finite element analysis code DIAL was modified to analyze this phenomena in two stages. First, the temporal moisture partial pressures in the materials are calculated from initial and environmental boundary conditions and second, these partial pressures are then passed to a quasi-static mechanical analysis where the moisture data at each time step is treated as a dilatation (swelling) load. The resulting stress and strain state is then calculated, as well as gap closure, if any.Demonstration of this methodology was on a relatively simple 2-dimensional axisymmetric example problem. The results show primarily compressive stresses caused by the positive dilatation (swelling) induced by moisture intrusion of the structure. The stresses concentrate at adhesive bondlines, and bondline gap behavior (i.e., the absence of a contiguous adhesive bond at material interfaces) is correctly simulated. It should be noted that the behavior of the gap and any resultant stress/strains due to a change in moisture is problem dependent, and this example problem is meant as a demonstration of methodology
